Club Ice Schedule and Contract for 2007/2008

• All Skaters must have passed Basic 6 or be in a lesson with a Coach to skate on Club Ice.
• There will be a $25 fee to make a permanent change for a contracted Session.
• Make-up slips will only be issued when a request is made to (978) 777-1974 before the start of the session.
• *Club Members Only* Special Rate – contract for two sessions from 3:15 to 5:05 on Tuesday or Friday and pay only $20 for both sessions – $ave $ and $k8 with our Club!

Agreement

This is a contract between you and Silver Streaks Skating Club for ice time from September 10, 2006 to June 22, 2007.  By accepting the terms of this contract, the skater and their parent/guardian is responsible for the full annual amount of the ice contract.  You will be billed from September to April in eight equal payments.  To confirm your commitment to contract ice, you will need complete this form below and send a deposit of $100 per session reserved ($200 for 2 sessions, etc.).  All invoices must be paid within 30 days of billing.
